Aveanna Healthcare is seeking a compassionate and driven Home Health Occupational Therapist to join our dedicated team of healthcare professionals. This role is pivotal in helping patients with diverse medical conditions to enhance their ability to perform daily activities and achieve their rehabilitation goals in the comfort of their homes. As a key member of our therapy team, this full-time position involves working closely with patients of various age ranges, primarily focusing on pediatric cases, developing tailored therapy plans, and collaborating with family members and other healthcare professionals to deliver holistic care. Conduct patient evaluations to determine their condition and needs. Develop and implement tailored therapy plans based on patients' physical, social, and emotional needs. Regularly reassess treatment plans and the effectiveness of interventions, adjusting as necessary. Educate patients and their families on treatments and-home-care techniques necessary for their progress. Document patient care and progress thoroughly and accurately in compliance with local, state, and federal laws. Collaborate with multidisciplinary team members to provide consistent care and communicate patient needs. Maintain confidentiality and respect for patient privacy at all times. Strong interpersonal and communication skills Proficiency in evidence-based therapy practices Ability to work independently and as part of a team In-depth understanding of patient-centered care Excellent organizational and time-management skills Medical Insurance Dental Insurance Vision Insurance 401k Active state-issued license as an Occupational Therapist NBCOT certification preferred Valid driver's license and reliable transportation CPR and Basic Life Support certifications
