Proficiency in HomeCare HomeBase EMR - Proficiency in Oasis
Description: Provides, coordinates, and directs the provision of
home nursing care according to physicians' orders. Care is based on
agency policies and procedures through the competent application of
the nursing process. Responsible to the Clinical Manager. Directly
supervises and evaluates Home Health Aides and Licensed Practical
Nurses/Licensed Vocational Nurses. The role involves home visits
and periodic office meetings. Reliable transportation, a current
driver’s license, and agency-required liability insurance are
necessary. Average number of patients per day is 5-8. Contractors
will be assigned between 5-10 patients per day that equate to
roughly 6-8 points. Types of cases include Start of Care, Wound
Care, Follow-up Visits, etc. Recent specialty experience is
required. Weekend requirements vary per location and profession.
Territory mileage radius requires willingness to travel up to 75
miles from the home clinic. Mileage will be reimbursed according to
the IRS standard rate. Local candidates are accepted at the same
rate. Pending licenses are not acceptable; candidates must have a
license in hand. There is no on-call requirement for this position.
Candidates must be open to traveling and accept all patients on
their schedule. Documentation must be completed within 24 hours of
the appointment with patients. All documentation impacts billing
and follow-up patient care. There are no guaranteed hours with the
contract. The expectation is to hit weekly productivity points of
30, which equates to 40 hours per week. If not receiving enough
patients each week, the option to float to another nearby clinic is
