Job Title: Case Manager RN Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum 2 years of RN Case Management experience, including experience in a long-term acute care setting License: Kentucky RN or multi-state RN license required Certifications: BLS certification from the American Heart Association required Must-Have: - 2 years of RN Case Management experience - Experience in discharge planning, clinical case management, and utilization review Description: Coordinates management of care and ensures optimum utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning. Implements care plans and evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through the integration of functions of case management and utilization review. Acts as a patient advocate through resource utilization and discharge planning. Addresses the holistic needs of the patient. Ensures patient preferences are taken into account.
